Mistakes happen. So do almost mistakes (otherwise called near misses or close calls or close shaves).
And many preventable mistakes happen because many almost mistakes happened before and no one said anything about them.
Just as mistakes are feedback and part of the process of learning and innovation, so are almost mistakes—that is if we get in the habit of acknowledging and reflecting on them.
If you want to accelerate learning and innovation while making fewer preventable mistakes, then cultivate a culture that incentivizes surfacing and talking about almost mistakes.
